Application for Parchment Reprint (PDF) - Use this form to replace a parchment that has been lost, stolen, or damaged.
Change of Name Form (PDF) – Submit this form when requesting a reprint of your degree due to change of name.

Service Request Form – The Service Request Form is to be used if your registration status in a term is designated as off-campus and you would like to request Athletics and Recreation, and/or U-Pass.
Postgraduate Medical Education Trainees (PGME) Service Request Form – The Service Request Form is to be used if you would like to request Athletics and Recreation, and/or U-Pass. If you would like to request the U-Pass you must be a member of the GSA and must opt-in to the GSA for the Fall and Winter Terms and the U-Pass concurrently. In order to be eligible for Spring/Summer U-Pass, you must have opted in to the GSA and U-Pass for the previous Fall and/or Winter term(s).
U-Pass Service Exemption Form – If you are eligible, use this form to request an exemption from the U-Pass. See Exemption Criteria for more information.
